Please read the full list of instructions for completing the fire academy application. Your application will not be considered until all steps have been completed.

Download the Requirements and Expectations form.
Download the Waiver, Release of Liability, and Assumption of Risk form.
Download the Physical Evaluation form.
Complete and submit the application below.
Once you have submitted your application, sign all completed forms and email to admin@futurefireacademy.org along with your prerequisite certificates. We recognize that there may be a wait period to obtain an appointment for your physical evaluation. Please provide all other documentation ASAP and provide the date of your appointment.
